REQUIREMENTS FOR ADMISSION

1. Original copy & photocopy of birth certificate with registration number.

2. Scholastic Performance Record(photocopy of report card).

3. Two(2) pcs. 2"x2" Recent Picture.

4. Certificate of Good Moral Character.

5. Accomplished Recommendation Form (to be accomplished by the applicant's current School Guidance Counselor and Adviser).

6. ACR (Alien Certificate Registration) for foreign students only.

APPLICATION PROCEDURE

1. Please proceed to the Admin's Office (Window 3 Registrar) for admission procedures and other inquiries.

2. For assessment of fees please proceed to (Window 2 Cashier).

3. Submit the duly accomplished Student Data Form together with the initial requirements(copy of birth certificate, pictures, and copy of report card)for verification by the Admission Officer.
